USC strives to create and maintain an accessible and welcoming environment for individuals with disabilities.
This form can be used by the USC community to report various accessibility barriers across the university, including instructional, technological or physical, that limit access to the university’s facilities, programs, academics, or digital footprint.
Please fill in as much information as you can regarding your concern. This form is reviewed by the Office of Institutional Accessibility and ADA Compliance during normal business hours.
In the event of an emergency, please call Campus Security or dial 911. Complete this form after reporting the emergency.
